Children:
Mild to moderate infections: 5-10 mg/kg/dose PO/IV Q6-8H
Severe infections, MRSA, Osteomyelitis: 10 mg/kg/dose PO/IV Q6H or 13 mg/kg/dose PO/IV Q8H
Adults:
150-600 mg/dose PO Q6-8H.
150-900 mg IV Q6-8H.
Max 2700 mg/24 hr

Caution: pseudomembranous colitis may occur at anytime during or several weeks after discontinuing therapy
