Dosages

Mild to Moderate Infections:  
IV: 25-50 mg/kg/dose IV Q6H Max 2000 mg/dose

Severe Infections:  
50 mg/kg/dose IV Q4-6H
Max 2000 mg/dose

Cystic Fibrosis:
50 mg/kg/dose IV Q4-6H. Max 2000 mg/dose

Adults:  
1-2 g/dose IV Q4-6H

Comments

Oral cloxacillin is poorly absorbed (bioavailability ~50%, reduced by food).

Consider using cephalexin as oral-stepdown in place of oral cloxacillin, as cephalexin has higher bioavailability, is more palatable and causes less GI upset.
