Dosages
Children:
Oropharyngeal or esophageal candidiasis: 6 mg/kg/dose PO once daily x 1. Maximum: 200 mg/dose), then 3 mg/kg/dose PO once daily (Maximum: 100 mg/dose)
UTI: 3-6 mg/kg/dose PO/IV once daily Maximum: 400 mg/dose
Invasive fungal infections:
12 mg/kg/dose PO/IV once daily Maximum: 800 mg/dose
Fungal Prophylaxis: 6-12 mg/kg/dose PO/IV once daily
Maximum: 400 mg/dose
Adults:
100-400 mg PO/IV once daily
Invasive fungal infections: 800 mg PO/IV once daily
Uncomplicated vulvovaginal candidiasis: 150 mg PO single dose

Comments
Bioavailability >90%.
May cause QT prolongation. Caution in patients with proarrythmic condition or renal and hepatic dysfunction. Use with caution if on other QT prolonging medications.
Dosage adjustment required in renal impairment.
Good CSF and vitreous penetration; achieving concentrations of > 50% of those in serum.
Affects CYP450 resulting in multiple drug interactions. May increase effects/levels of cyclosporine, phenytoin, vincristine, warfarin and zidovudine. Consult pharmacy prior to starting any drug therapy.
