Standard Prescription
L-arginine__g PO__(frequency) (__g/kg/24hr) or (__g/m2/24hr)
L-arginine __g IV (__g/kg/dose) x 1 loading dose
L-arginine__g IV Q__H (__g/kg/dose) or (__g/m2/24hr)

Dosages
Oral:
Weight < 20 kg: 100-700 mg/kg/24 hr added to daily formula
Weight > 20 kg: 2.5-15.4 g/m 2 /24 hr PO added to daily formula.
Max 6 g/day
I.V.:
Weight < 20 kg:
Load: 200-600 mg/kg/dose IV over 90 min.
Maintenance: 200-600 mg/kg/day by continuous IV infusion.
Weight > 20 kg:
Load: 3.6-7.2 g/m 2 /dose IV over 90 min.
Maintenance: 3.6-7.2 g/m 2 /dose by continuous IV infusion over 24 hours.
Max 12 g/day
Acute Stroke Like Episodes (MELAS):
250 mg/kg/dose IV BID-QID Max: 1000 mg/kg/day
Pituitary Function Test:
0.5 g/kg/dose

Comments
If total daily dose is not added to the child's daily formula, then it may be divided BID-TID.
for continuous IV infusion, L-arginine is combined in the same IV bag as sodium benzoate and sodium phenylacetate in D10W.
pre-treatment with IV ondansetron prior to IV loading dose is recommended; followed by PRN doses.
supplemental potassium may be required; monitor.
KCl is usually added to the continuous IV infusion mixture at 20 mmol/L.
