Standard Prescription

mycophenolate mofetil__mg PO/IV Q__H (__mg/m2/dose)
mycophenolate sodium__mg PO Q__H (__mg/m2/dose)

Dosages

Kidney Transplant:
Mycophenolate mofetil:
600 mg/m2/dose PO/IV BID
Maximum: 1000 mg/dose

Mycophenolate sodium (Myfortic®):
432 mg/m2/dose PO BID (max: 720 mg BID)
BSA 1.19-1.58 m2: 540 mg PO BID
BSA > 1.58 m2: 720 mg PO BID

Nephrotic Syndrome:
600 mg/m2/dose BID
Maximum: 1000 mg/dose

GVHD Prophylaxis for HSCT
15 mg/kg/dose IV Q8H. Maximum 1000 mg/dose
Once able to tolerate oral, switch to 15 mg/kg/dose PO BID-TID. Maximum 1000 mg/dose

Cellcept suspension (mycophenolate mofetil):
200 mg/mL

Myfortic (mycophenolate sodium) EC tablet:
180 mg, 360 mg

injection (mycophenolate mofetil):
500 mg/vial

Comments

Mycophenolate mofetil 1000 mg = mycophenolate sodium (Myfortic) 720 mg.

Myfortic
 tablets are enteric coated; do not crush, chew or split the tablets.

Teratogenic: women of reproductive potential must be counselled on pregnancy planning and prevention.

Other organ transplants (heart, liver, lung, bowel, etc,) follow specific protocols.
