basiliximab __mg IV x 1 dose (__mg/m2/dose)
Dosages
Kidney Transplant Induction:
Day 0 and Day 4:
12 mg/m2/dose IV. Minimum: 10 mg/dose. Maximum 20 mg/dose
Give 4 hours pre-op on Day 0
May also dose by weight category:
<35 kg: 10 mg IV
>=35 kg: 20 mg IV

Comments
Medications for treatment of severe hypersensitivity reactions should be available for immediate use.
Patients are not usually given a second course of basiliximab for transplant induction due to the risk of severe allergic reaction/anaphylaxis, but each case must be considered on an individual basis.
